Enrolment application

To apply for enrolment in the Foundations in Mental Health Practice, you will be required to: 

1. Complete and sign the form below 
2. Complete a Language, Literacy and Numeracy quiz 
3. Complete a Digital Readiness quiz 

Please note this is an application only and your enrolment in the course is not guaranteed. Incomplete applications will not be considered. 

Students can withdraw their enrolment at any stage. To do so, please submit your request to rto.student@orygen.org.au. 

Please ensure you have accessed and read the course outline and the student handbook prior to submitting your enrolment form.

About the quizzes: 

The quiz links will be emailed to you once you verify your email address. These quizzes are to ensure your level of language, literacy, numeracy and digital skills are at a level where you will be able to engage with the course comfortably. If an issue arises with your quiz results, Orygen may contact you to discuss your options, including a student support plan.
